Who are we?
The Anticoagulation Forum is a network of physicians, nurses, pharmacists, and other health care providers who staff anticoagulation clinics. There are more than 3000 members who represent over 1100 different anticoagulation management services (clinics)throughout the United States.
What are the Accomplishments of the Forum?
Over the last eleven years the Forum has actively promoted the concept of coordinated management of oral anticoagulation through the model of an anticoagulation management service or anticoagulation clinic. It has established guidelines for the development of anticoagulation management services, has promoted the appropriate monitoring of anticoagulation through the use of International Normalized Ratio, has encouraged the expanded use of anticoagulation for established indications (atrial fibrillation), has developed guidelines for the certification of anticoagulation providers, and has lobbied third-party payers to appropriately reimburse healthcare providers for the management of therapy.

How much does it cost to join?
There is no cost to join the Forum at this time. It is funded by unrestricted educational grants from the pharmaceutical and diagnostic devices industries.
How do I join?
You may join the Anticoagulation Forum by completing one of the two Registration Form in the "Join The Forum" section. If you are a professional actively working in an anticoagulation clinic, private office, hospital or educational facility, please complete Active Membership Enrollment Form. If you are in the pharmaceutical field, involved in diagnostic devices, in health care management, or you are a consultant in the field of anticoagulation therapy, please complete the Corporate Membership Enrollment Form.
How do I know I have been enrolled?
You will be notified within a short time of your enrollment in the Anticoagulation Forum by mail, confirmed fax or e-mail.
